Insurance

Creating Nutrition will file claims with contracted insurance providers only. A referral from your doctor is required to bill insurance. Referrals can be faxed to 541-224-5277. We are a Medicaid provider and do not with file Medicare. Payment is due at time of service if insurance is not filed.

If using insurance, it is your responsibility to verify insurance coverage. To help avoid any unanticipated out of pocket responsibilities we recommend contacting your insurance company prior to scheduling your appointment. Questions to ask:

• Are you covered for Medical Nutrition Therapy? CPT code 97802 and 97803.

• Is your coverage limited to a certain number of visits?

• Have you met your deductible? Until your deductible is met you will be charged in full for services rendered.

• Do you have any Co-pays? Co-pays will be due at the time of service.
